A typical rash accompanied by fever occurs in the emergency department mainly in connection with childhood infectious diseases. With regard to the increased migration from the east and south and taking into account the possible evaporation of specific immunity, we encounter these diseases even in adult patients. Here, the course and sowing are more often atypical and we rely more on laboratory diagnostic methods. Of the macular and maculopapular exanthemas, we most often encounter the fifth and sixth diseases, also measles or scarlet fever. Vesiculous exanthemas include varicella and hand-foot-mouth syndrome. Especially in viral exanthematous diseases, we are more often afraid of bacterial complications than the primary disease itself. In some cases, infection of a pregnant woman is significantly dangerous. First of all, this is varicella.

Key words: fever – exanthema – emergency department – measles – varicella

MODIFICATIONS´ PROPOSAL TO IMPROVE THE USABILITY OF THE FUNCTION OF VIDEO CALLS TO MEDICAL DISPATCH CENTERS

Gabriela Divišková, František Němec, Jan Mužík

Abstract

The frequency of use of the video call function in the Czech Republic is very low compared to foreign countries. Pilot inquiry concentrated on the use of video call from mobile applications for summoning help by medical operation centres in the Czech Republic [1] has shown that the frequency of activation of the function is in the order of units of attempts to make a video call in a whole year. Compared to Norway, where video calls account for up to 10 % of all emergency calls, is it very low and other problems have been identified, too. These include number of steps callers have to take to activate the funciton, poor signal coverage in some areas, low interest in the technology from dispatchers, especially for not expecting beneficial information from video. Area which hasn´t been solved yet is the legislatition as video calls are not recorded or archived. Based on the analysis of the collected data, modifications to the systems functionality were proposed and submitted to the dispatchers for comments. The evaluation of the changes was carried out in the form of structured questionnaires, which revealed the need of dispatcehrs to expand their awareness of the benefits of the function, further education and establishing a space for sharing experience within the Czech Republic and foreign countries. Furthermore, there is a need to reduce the steps needed, i.e. to consider the possibility of automatically triggering the speakerphone and allowing access to the camera already when starting the video call. Last but not least, it is necessary to point out the need for legislative changes in this area to ensure a safe environment on the side of the dispatcher and the caller, too.

Key words: Video call – medical dispatch center – telephone-assisted cardiopulmonary resuscitation

THE CONCEPT OF OXYGEN DELIVERY – THE PHYSIOLOGICAL BASIS OF THE TREATMENT OF CRITICAL CONDITIONS

Vladimír Černý, David Astapenko, David Řehák

Abstract

The clinical physiology of oxygen delivery is one of the cornerstones in the field of acute medicine. With the help of the program, we can analyze the interventions and provide feedback to doctors and paramedics. The data recorded by the Lifepak 15 monitor during EMS run are remotely transferred to a computer, where the software enables the analysis of individual aspects of the intervention on the timeline, such as the ECG curve, capnography, oxygen saturation and other unmeasured values. The obtained data can also be very well used in CPR teaching.
Key words: cardiopulmonary resuscitation (CPR) – feedback – education in prehospital emergency care

LANGUAGE BARRIER IN PRE-HOSPITAL EMERGENCY CARE

Jaroslav Pekara, Eva Čižmárová

Abstract

Language barrier in the provision of pre-hospital emergency care is the topic of the presented paper. We provided a quantitative survey (questionnaire survey; n=104) to determine which foreigners emergency medical service personnel encounter most frequently and what foreign languages they themselves speak. Our respondents most frequently encounter Ukrainians and the most common foreign language of respondents is English. The secondary goal of the study was to find out how the management of the ambulance service approaches the issue of communication with patients of other nationalities. The results of the survey indicated that the management of the ambulance service tries to help its staff (printed materials, communication cards and telephone interpretation). A crucial result of the survey was the finding that ways of overcoming the language barrier are not fully used by the staff in the pre-hospital emergency care.
Key words: foreigners – language barrier – communication – pre-hospital emergency care

INTRODUCTION Contents Editorial – Jana Šeblová EMERGENCY DEPARTMENTS Patient with fever and rash at the Enmergency Department – Robin Šín, Dalibor Sedláček SYSTEMS´ ORGANIZATION AND MANAGEMENT Modifications´ proposal to improve the usability of the function of video calls to medical

[22
