C\u00edlem t\u00e9to studie bylo zjistit, zda transport hemodynamicky stabiln\u00edho pacienta s polytraumatem p\u0159\u00edmo z dopravn\u00edho prost\u0159edku ZZS na celot\u011blov\u00e9 CT m\u016f\u017ee statisticky v\u00fdznamn\u011b zkr\u00e1tit \u010das do dokon\u010den\u00ed CT vy\u0161et\u0159en\u00ed v porovn\u00e1n\u00ed se standardn\u00edm postupem, kdy je pacient p\u0159evzat od pos\u00e1dky ZZS na odd\u011blen\u00ed urgentn\u00edho p\u0159\u00edjmu.<br \/>\nMetoda: V t\u00e9to studii jsme hodnotili \u010das do dokon\u010den\u00ed celot\u011blov\u00e9ho CT (\u010das ven\u00f3zn\u00ed f\u00e1ze kontrastn\u00edho vy\u0161et\u0159en\u00ed) u dvou skupin hemodynamicky stabiln\u00edch a intubovan\u00fdch pacient\u016f s polytraumatem. V prvn\u00ed skupin\u011b byl pacient transportov\u00e1n z dopravn\u00edho prost\u0159edku ZZS (v\u016fz nebo vrtuln\u00edk) v doprovodu t\u00fdmu ZZS a nemocni\u010dn\u00edho trauma t\u00fdmu p\u0159\u00edmo na CT vy\u0161et\u0159en\u00ed, kde prob\u011bhlo p\u0159ed\u00e1n\u00ed pacienta. Ve druh\u00e9 skupin\u011b byl pacient transportov\u00e1n na odd\u011blen\u00ed urgentn\u00edho p\u0159\u00edjmu, kde prob\u011bhlo p\u0159evzet\u00ed pacienta a n\u00e1sledn\u011b byl pacient transportov\u00e1n na vy\u0161et\u0159en\u00ed CT v doprovodu nemocni\u010dn\u00edho trauma t\u00fdmu.<br \/>\nV\u00fdsledky: Analyzovali jsme data 62 pacient\u016f. Pr\u016fm\u011brn\u00fd \u010das od p\u0159evzet\u00ed pacienta od pos\u00e1dky ZZS do proveden\u00ed celot\u011blov\u00e9ho CT vy\u0161et\u0159en\u00ed byl v prvn\u00ed skupin\u011b (21 pacient\u016f) 10\u00b1 2,3 minut (min 6, max. 14 minut). Ve druh\u00e9 skupin\u011b (41 pacient\u016f) byl tento \u010das signifikantn\u011b del\u0161\u00ed 31 \u00b1 12,6 minuty (min 15, max. 59 minut). Tento rozd\u00edl byl statisticky v\u00fdznamn\u00fd (p &lt;0,0001).<br \/>\nZ\u00e1v\u011br: Transport hemodynamicky stabiln\u00edho pacienta s polytraumatem p\u0159\u00edmo na CT vy\u0161et\u0159en\u00ed v doprovodu pos\u00e1dky ZZS a nemocni\u010dn\u00edho trauma t\u00fdmu zkracuje \u010das do definitivn\u00ed diagnostiky potenci\u00e1ln\u011b \u017eivot ohro\u017euj\u00edc\u00edch poran\u011bn\u00ed.<\/p>\n<p><b>Kl\u00ed\u010dov\u00e1 slova: <\/b>celot\u011blov\u00e9 CT vy\u0161et\u0159en\u00ed \u2013 E-FAST \u2013 polytrauma<\/p>\n<p><b>Abstract <\/b><\/p>\n<p>Introduction: According to recent studies, early diagnosis of lifethreatening injuries using CT scans in patients with polytrauma can reduce their mortality. For this reason, it is important to look for ways to improve the management of polytrauma patient admission and reduce the time from patient admission to completion of CT scans. The aim of this study was to determine whether the transport of a hemodynamically stable patient with polytrauma directly from an EMS ambulance to a whole-body CT can statistically significantly shorten the time to completion of CT examination compared to the standard procedure where the patient is taken from the paramedic crew to the emergency department.<br \/>\nMethod: In this study, we evaluated the time to completion of whole-body CT (time of venous phase of contrast examination) in two groups of hemodynamically stable and intubated patients with polytrauma. In the first group, the patient was transported from the paramedic vehicle (car or helicopter) accompanied by the paramedic team and the hospital trauma team directly to the CT examination, where the patient was handed over. In the second group, the patient was transported to the emergency department, where the patient was taken over and subsequently the patient was transported to a CT examination accompanied by a hospital trauma team.<br \/>\nResults: We analyzed data from 62 patients. The average time from taking the patient from the emergency medical crew to performing a whole-body CT examination in the first group (21 patients) was 10 \u00b1 2.3 minutes (min 6, max 14 minutes). In the second group (41 patients), this time was significantly longer 31 \u00b1 12.6 minutes (min 15, max 59 minutes). However, several ultrasound findings which may reduce the reliability of the examination have been documented. Not only the EtCO2 value itself is beneficial, but also its graphical representation over time \u2013 the capnographic curve. Capnometry provides useful information about the patient&#8217;s cardiopulmonary apparatus: adequate minute lung ventilation, cardiac output, airway obstruction, and dead ventilation. Theoretical teaching, practice of life-saving procedures and demonstration of morphological findings in forensic medicine has provided students from the IRS for several years with a comprehensive view of some injury mechanisms, life-saving procedures and dead body handling.
