Autorka v kazuistice prezentuje p\u0159\u00edpad mlad\u00e9 \u017eeny, kter\u00e1 byla opakovan\u011b vy\u0161et\u0159ena a hospitalizov\u00e1na pro projevy anemick\u00e9ho syndromu p\u0159i t\u011b\u017ek\u00e9 normocyt\u00e1rn\u00ed normochromn\u00ed sideropenick\u00e9 an\u00e9mii. Pacientka byla vy\u0161et\u0159ov\u00e1na na n\u00edzkoprahov\u00e9m intern\u00edm p\u0159\u00edjmu a hospitalizov\u00e1na na okresn\u00edm intern\u00edm i specializovan\u00e9m hematologick\u00e9m odd\u011blen\u00ed. Absolvovala mno\u017estv\u00ed neinvazivn\u00edch i invazivn\u00edch vy\u0161et\u0159en\u00ed, av\u0161ak ke spr\u00e1vn\u00e9 diagn\u00f3ze vedla a\u017e d\u016fkladn\u011b odebran\u00e1 a ov\u011b\u0159en\u00e1 anamn\u00e9za. Kazuistika poukazuje na \u00faskal\u00ed diferenci\u00e1ln\u00ed diagnostiky an\u00e9mie a poruch p\u0159\u00edjmu potravy. Patologick\u00e9 v\u00fdsledky specializovan\u00fdch vy\u0161et\u0159en\u00ed bez komplexn\u00edho p\u0159\u00edstupu k pacientovi mohou b\u00fdt zav\u00e1d\u011bj\u00edc\u00ed. Objasn\u011bn\u00ed kauz\u00e1ln\u00ed p\u0159\u00ed\u010diny obt\u00ed\u017e\u00ed je nezbytn\u00fdm p\u0159edpokladem c\u00edlen\u00e9 l\u00e9\u010dby. Ur\u010den\u00ed spr\u00e1vn\u00e9 diagn\u00f3zy u t\u00e9to pacientky vedlo k adekv\u00e1tn\u00ed, v tomto p\u0159\u00edpad\u011b psychiatrick\u00e9 l\u00e9\u010db\u011b s velmi nejistou progn\u00f3zou.<\/p>\n<p><b>Kl\u00ed\u010dov\u00e1 slova: <\/b>sideropenick\u00e1 an\u00e9mie \u2013 venepunkce \u2013 porucha p\u0159\u00edjmu potravy \u2013 ment\u00e1ln\u00ed bulimie \u2013 ment\u00e1ln\u00ed anorexie \u2013 hypoglykemia factitia \u2013 M\u00fcnchhausen\u016fv syndrom<\/p>\n<p><b>Abstract <\/b><\/p>\n<p>Case report of a young woman who was repeatedly examined and hospitalized for manifestations of anaemic syndrome in severe normocytic normochromic sideropenic anaemia. The patient was evaluated in a low-threshold internal medicine admission and hospitalized in the district internal medicine and specialized hematology departments. She underwent a large number of noninvasive and invasive investigations, but only a thoroughly taken and verified case history led to the correct diagnosis. The case report highlights the pitfalls of differential diagnosis of anaemia and eating disorders. Pathological results of specialized examinations without a comprehensive approach to the patient can be misleading. Elucidation of the causal cause of the difficulties is a prerequisite for targeted treatment. Establishing the correct diagnosis in this patient led to adequate, in this case psychiatric treatment with a very uncertain prognosis.<\/p>\n<p><b>Key words: <\/b>sideropenic anaemia \u2013 venepuncture \u2013 eating disorder \u2013 mental bulimia \u2013 mental anorexia \u2013 hypoglycemia factitia \u2013 M\u00fcnchausen syndrome<\/p>\n<h6><b>BEZPE\u010cN\u00c9 PROST\u0158ED\u00cd ZDRAVOTNICK\u00c9HO Z\u00c1CHRAN\u00c1\u0158E<\/b><\/h6>\n<p><b>SAFE ENVIRONMENT FOR PARAMEDICS<\/b><\/p>\n<p><b>Jaroslav Pekara, Ond\u0159ej Br\u016f\u017eek<\/b><\/p>\n<p><b>Abstrakt <\/b><\/p>\n<p>T\u00e9matem sd\u011blen\u00ed je zmapov\u00e1n\u00ed rizik, se kter\u00fdmi se mohou setkat pracovn\u00edci zdravotnick\u00e9 z\u00e1chrann\u00e9 slu\u017eby. \u010cl\u00e1nek se zam\u011b\u0159uje na rizika spojen\u00e1 s nep\u0159ehledn\u00fdm ter\u00e9nem a sleduje mo\u017en\u00e1 nebezpe\u010d\u00ed od okam\u017eiku p\u0159ijet\u00ed v\u00fdzvy na opera\u010dn\u00edm st\u0159edisku a\u017e po p\u0159ed\u00e1n\u00ed pacienta v nemocnici. C\u00edlem sd\u011blen\u00ed je snaha popsat co nejucelen\u011bj\u0161\u00ed p\u0159ehled v\u0161ech rizik, se kter\u00fdmi se pracovn\u00edci ZZS mohou setkat. \u010cl\u00e1nek se v\u011bnuje z\u00e1kladn\u00edm informac\u00edm t\u00fdkaj\u00edc\u00edm se nej\u010dast\u011bj\u0161\u00edch infekc\u00ed, se kter\u00fdmi se pos\u00e1dky ZZS p\u0159i z\u00e1sahu setk\u00e1vaj\u00ed, a z\u00e1kladn\u00edm osobn\u00edm ochrann\u00fdm pracovn\u00edm pom\u016fck\u00e1m. \u010cl\u00e1nek p\u0159ipojuje i pr\u016fzkum, kter\u00fd zjistil, \u017ee pracovn\u00edci ZZS v\u017edy nevyu\u017e\u00edvaj\u00ed pot\u0159ebn\u00e9 a dostupn\u00e9 ochrann\u00e9 prost\u0159edky. Praktick\u00e1 \u010d\u00e1st t\u00e9to pr\u00e1ce si vymezila dva c\u00edle, a to zjistit, jak\u00e9 je pov\u011bdom\u00ed zdravotnick\u00fdch z\u00e1chran\u00e1\u0159\u016f o nebezpe\u010dn\u00fdch faktorech ovliv\u0148uj\u00edc\u00edch jejich pr\u00e1ci, a d\u00e1le zjistit, zda pracovn\u00edci ZZS v\u017edy vyu\u017e\u00edvaj\u00ed pot\u0159ebn\u00e9 a dostupn\u00e9 ochrann\u00e9 prost\u0159edky.<\/p>\n<p><b>Kl\u00ed\u010dov\u00e1 slova: <\/b>bezpe\u010d\u00ed \u2013 biologick\u00e1 rizika \u2013 nep\u0159ehledn\u00fd ter\u00e9n \u2013 zdravotnick\u00fd z\u00e1chran\u00e1\u0159<\/p>\n<p><b>Abstract <\/b><\/p>\n<p>The article deals with mapping the risks that can be faced by emergency medical services. It focuses on the risks associated with disorganized terrain and traces the possible hazards from the moment a call is received at the medical dispatch until the patient is handed over at the hospital. The aim of the communication is to provide as comprehensive overview as possible of all the risks that ambulance workers may face. The article discusses basic information on the most common infections encountered by paramedics and basic personal protective equipment. The article also includes a survey showing that EMS personnel do not always use the necessary and available protective equipment. Chyby spojen\u00e9 s poskytov\u00e1n\u00edm zdravotn\u00ed p\u00e9\u010de jsou z velk\u00e9 \u010d\u00e1sti preventabiln\u00ed. Ke sn\u00ed\u017een\u00ed bezpe\u010dnosti na urgentn\u00edm p\u0159\u00edjmu p\u0159isp\u00edv\u00e1 n\u00e1razov\u00e9 p\u0159et\u00ed\u017een\u00ed, p\u0159eru\u0161ov\u00e1n\u00ed p\u00e9\u010de a multitasking, ru\u0161n\u00e9 prost\u0159ed\u00ed, komplexita pacient\u016f, \u010dasov\u00fd tlak a nutnost st\u00e1l\u00e9ho rozhodov\u00e1n\u00ed a s t\u00edm spojen\u00e1 i zodpov\u011bdnost. Bezpe\u010dnost poskytovan\u00e9 p\u00e9\u010de je jedn\u00edm z kl\u00ed\u010dov\u00fdch prvk\u016f kvality. Je pot\u0159eba analyzovat data o ne\u017e\u00e1douc\u00edch ud\u00e1lostech a bezpe\u010dnostn\u00edch incidentech a na z\u00e1klad\u011b anal\u00fdzy chyb prov\u00e9st preventivn\u00ed opat\u0159en\u00ed. Medical errors associated with providing care are in most cases preventable. Overcrowding, frequent disruption of care, multitasking, busy environment, complexity of cases, time pressure and continuous decision-making together with high responsibility are some of the reasons which decrease the safety at emergency departments. Safety of the provided care is one of key elements of quality. It is necessary to analyze the data on adverse events and safety incidents and take preventive measures based on this analysis.
