Ta zaji\u0161\u0165uje dostate\u010dnou podporu ob\u011bhu, a tedy i org\u00e1novou perfuzi. ECPR poskytuje \u010das k diagnostice a l\u00e9\u010db\u011b reverzibiln\u00edch p\u0159\u00ed\u010din srde\u010dn\u00ed z\u00e1stavy a je vyu\u017e\u00edv\u00e1na u nemocni\u010dn\u00edch(IHCA) i mimo nemocni\u010dn\u00edch (OHCA) srde\u010dn\u00edch z\u00e1stav. \u00dasp\u011b\u0161nost metody vy\u017eaduje p\u0159\u00edsn\u00e1 krit\u00e9ria pro v\u00fdb\u011br kandid\u00e1t\u016f. Sou\u010dasn\u00e9 studie ukazuj\u00ed, \u017ee ECPR m\u016f\u017ee zlep\u0161it p\u0159e\u017eit\u00ed u pacient\u016f s OHCA p\u0159ibli\u017en\u011b na 30 % ve srovn\u00e1n\u00ed s konven\u010dn\u00ed KPR, jej\u00ed\u017e \u00fasp\u011b\u0161nost se pohybuje kolem 10 %. Progn\u00f3za pacient\u016f je v\u00fdrazn\u011b ovlivn\u011bna v\u011bkem, d\u00e9lkou isch\u00e9mie nebo nap\u0159\u00edklad typem srde\u010dn\u00ed z\u00e1stavy. V n\u00e1sleduj\u00edc\u00edm textu p\u0159edstavujeme p\u0159ehled indikac\u00ed, praktick\u00fdch aspekt\u016f a faktor\u016f, kter\u00e9 jsou podkladem k rozhodov\u00e1n\u00ed o p\u0159\u00edpadn\u00e9m zah\u00e1jen\u00ed ECMO podpory. Obecn\u011b lze konstatovat, \u017ee ECPR m\u016f\u017ee b\u00fdt efektivn\u00ed u pacient\u016f do 70 let v\u011bku se spat\u0159enou, p\u0159edpokl\u00e1danou kardi\u00e1ln\u00ed p\u0159\u00ed\u010dinou kolapsu, efektivn\u00ed konven\u010dn\u00ed KPR zah\u00e1jenou sv\u011bdky na m\u00edst\u011b a intervalem kolaps-ECMO do 60 minut.<\/p>\n<p><b>Kl\u00ed\u010dov\u00e1 slova: <\/b>extrakorpor\u00e1ln\u00ed membr\u00e1nov\u00e1 oxygenace \u2013 mimot\u011bln\u00ed kardiopulmon\u00e1ln\u00ed resuscitace \u2013 indika\u010dn\u00ed krit\u00e9ria<\/p>\n<p><b>Abstract <\/b><\/p>\n<p>It is possible to consider advanced resuscitation methods in the case of refractory cardiac arrest, extracorporeal cardiopulmonary resuscitation (ECPR) is one of them. It enables adequate circulatory support and organ perfusion. ECPR provides time for diagnostics and treatment of reversible causes of cardiac arrest and it is used bothin intra-hospital (IHCA) and out-of-hospital (OHCA) cardiac arrests. Strict rules for indication are necessary for its effectiveness. Current studies indicate improvement in survival rates up to 30 % compared to 10 % in standard CPR. Age, duration of ischaemia and type of cardiac arrest are among factors influencing patients\u00b4 prognosis. This paper presents the overview of indications, practical aspects and factors which have impact on decicion to start ECPR support. The article deals with the identification of palliatively relevant patients, their needs and symptoms and focuseson the potential pitfalls in providing care to these patients. The aim of the paper is to describe these pitfalls in providing care to palliative patients in the prehospital emergency care setting as perceived by paramedics. Key challenges include communication failures, diagnostic errors, emergency department crowding, andinefficient patient handovers. This article briefly identifies major safety issues in pre-hospital and hospital care and proposes strategies to improve safety. Furthermore, due tothe change in legislation in Act No. 96\/2004 Coll., on non-medical healthcare staff in \u00a7 18, from 2017 graduates from this field are required to complete at least one year of practice with providers of acute inpatient intensive care, which delays their entry into the profession of a paramedic at EMS\u2018s.
