Simula\u010dn\u00ed medic\u00edna je ve zdravotnictv\u00ed pom\u011brn\u011b nov\u00fd, dynamicky se rozv\u00edjej\u00edc\u00ed obor. Jedn\u00e1 se o modern\u00ed zp\u016fsob v\u00fduky za pomoci simul\u00e1toru, je\u017e nahrazuje pacienta. Simulation medicine is a modern dynamically developing way of teaching in healthcare using a patient simulator. The goal of this communication is to provide a comprehensive view on the possible use of this technology to improve healthcare with respect to the safety of patients.<\/p>\n<p><b>Key words: <\/b>non-technical skills \u2013 patient safety \u2013 simulation \u2013 simulation medicine<\/p>\n<p>&nbsp;<\/p>\n<p><b>EFEKTIVN\u00cd ROZLO\u017dEN\u00cd SIL DVOU ZACHR\u00c1NC\u016e B\u011aHEM RESUSCITACE: SIMULA\u010cN\u00cd STUDIE<\/b><\/p>\n<p><b>JINDRA HOLEKOV\u00c1, KAROL\u00cdNA KALU\u017dOV\u00c1, JAN POSP\u00cdCHAL<\/b><\/p>\n<p><b>Abstrakt <\/b><\/p>\n<p><em>V\u00fdchodiska:<\/em> Technika neodkladn\u00e9 resuscitace (KPR) se neust\u00e1le zdokonaluje. KPR je fyzicky n\u00e1ro\u010dn\u00e1 a prok\u00e1zala se \u00fa\u010dinnost spolupr\u00e1ce v\u00edce zachr\u00e1nc\u016f na m\u00edst\u011b. Pr\u016fzkum se zam\u011b\u0159uje na efektivitu rozlo\u017een\u00ed sil dvou zachr\u00e1nc\u016f p\u0159i KPR.<br \/>\n<em>C\u00edl:<\/em> Porovnat efektivitu z\u00e1kladn\u00ed neodkladn\u00e9 resuscitace prov\u00e1d\u011bn\u00e9 jedn\u00edm zachr\u00e1ncem st\u0159\u00eddan\u00fdm ve 2 minutov\u00fdch cyklech nebo dvojic\u00ed zachr\u00e1nc\u016f sou\u010dasn\u011b po celou dobu resuscitace.<br \/>\n<em>Metodika:<\/em> Simulace resuscitace se z\u00fa\u010dastnilo 40 respondent\u016f (20 dvojic student\u016f oboru Zdravotnick\u00fd z\u00e1chran\u00e1\u0159). \u010casov\u00fd limit KPR byl 10 minut. KPR byla prov\u00e1d\u011bna ve dvojici respondent\u016f, kte\u0159\u00ed se po dvou minut\u00e1ch st\u0159\u00eddali v prov\u00e1d\u011bn\u00ed kompletn\u00ed KPR. Po 60 minut\u00e1ch odpo\u010dinku n\u00e1sledovalo dal\u0161\u00edch 10 minut KPR, tentokr\u00e1t dvojice resuscitovala spole\u010dn\u011b po celou dobu. Hodnocena byla efektivnost a spr\u00e1vnost resuscitace, fyzick\u00e1 n\u00e1ro\u010dnost a subjektivn\u00ed pocity.<br \/>\n<em>V\u00fdsledky:<\/em> Celkov\u00e1 \u00fasp\u011b\u0161nost KPR u obou zp\u016fsob\u016f nebyla rozd\u00edln\u00e1 (96 % resp. 97 %). Dal\u0161\u00ed hodnocen\u00e9 parametry byly lep\u0161\u00ed p\u0159i prov\u00e1d\u011bn\u00ed KPR dv\u011bma zachr\u00e1nci sou\u010dasn\u011b. Vzestup tepov\u00e9 frekvence byl po KPR ve dvou zachr\u00e1nc\u00edch v\u00fdznamn\u011b ni\u017e\u0161\u00ed (p = 0,000), stejn\u011b tak i dechov\u00e9 frekvence (p = 0,000). V\u00fdskyt z\u010derven\u00e1n\u00ed v obli\u010deji nebyl v\u00fdznamn\u011b rozd\u00edln\u00fd (p = 0,084).<br \/>\n<em>Z\u00e1v\u011bry:<\/em> Kvalitn\u011b prov\u00e1d\u011bt resuscitaci je mo\u017en\u00e9 ob\u011bma zp\u016fsoby. Na z\u00e1klad\u011b m\u011b\u0159en\u00ed fyziologick\u00fdch funkc\u00ed i podle subjektivn\u00edho hodnocen\u00ed respondent\u016f je pro zachr\u00e1nce m\u00e9n\u011b n\u00e1ro\u010dn\u00e9 sou\u010dasn\u00e9 prov\u00e1d\u011bn\u00ed KPR ve dvou zachr\u00e1nc\u00edch.<\/p>\n<p><b>Kl\u00ed\u010dov\u00e1 slova: <\/b>Kardiopulmon\u00e1ln\u00ed resuscitace \u2013 efektivita \u2013 z\u00e1kladn\u00ed neodkladn\u00e1 resuscitace \u2013 prvn\u00ed pomoc<\/p>\n<p><b>Abstract <\/b><\/p>\n<p><b>Effective power distribution of two rescuers during resuscitation: a simulation study<\/b><\/p>\n<p><em>Background:<\/em> The Basic life supprot (BLS) has been constantly improving. The BLS is physically demanding aktivity and has been proven the advantage of more bystanders on site. The article focuses on the effectiveness of the power distribution of two bystanders during BLS.<br \/>\n<em>Aim:<\/em> To determine the effectiveness of the BLS performed by one bystander changing in 2 minute intervals or two bystanders at the same time throughout the BLS.<br \/>\n<em>Methods:<\/em> The BLS simulation based prospective study. 40 participants (20 pairs of paramedic students) were involved in this study. First, BLS was performed by one bystander (changing cycles of rest and BLS after 2 minutes) for ten minutes and after 60 minutes of rest, was performed by two bystanders at the same time. The efficiency and correctness of resuscitation, physical exertion and subjective feelings were evaluated.<br \/>\n<em>Results:<\/em> Overall success rate of the BLS was not different (96% and 97% respectively) in both groups. The other evaluated parameters were slightly better when BLS was performed with two bystanders at the same time. The increase of heart rate was significantly lower (P = 0.000) after the BLS in two bystanders, as well as the respiratory rate (p = 0.000). The incidence of reddening in the face was not significantly higher (p = 0.084).<br \/>\n<em>Conclusions:<\/em> Properly performed BLS is possible using both ways. Causes of such a dissimilarity lie in objective locoregional specifics on one side and in different way of dispatching practice and management on the other side.
